Infertility means the inability to naturally conceive a child and fertility treatment is the medication and procedures undertaken by couples to help them become pregnant. Assisted Reproductive Technology (ART) refers not only to IVF (In Vitro Fertilisation) but also to the various fertility procedures which may be required. These are usually combined with conventional therapies like fertility drugs to increase success rates.

Investing in tests to establish fertility problems and in assisted reproductive technology is a very expensive business, so it is not surprising that couples are seeking fertility treatment abroad. Treatment is much less expensive in India than in the UK and 'reproductive tourism' as it is becoming known, is really growing. Doctors in India more often take a holistic approach to fertility problems, drawing on their knowledge and understanding of traditional Indian medical treatments like Ayurveda or naturopathy when dealing with patients.


Following are some of the options for couples to consider : -
  • Intra Uterine Insemination (IUI)
  • In Vitro Fertilisation (IVF) and Embryo Transfer (ET) or 'test tube baby'
  • IVF using a donor's eggs
  • Intra Cytoplasmic Sperm Injection (ICSI)
  • Assisted Hatching
  • Cryo Preservation of Embryos
  • Semen Bank - hospitals often have their own semen bank with frozen sperm from voluntary donors

Both fallopian tubes are absent or blocked due to surgery or tubal pregnancy) or infection (STD, or Tuberculosis) Endometriosis Reduced sperm count or motility (IVF can be normally performed for counts which are more than 5 million per ml. Procedure of In-Vitro-Fertilization

There are five major steps in the IVF and embryo transfer sequence : -
  • Monitor the development of ripening egg(s) in the ovaries
  • Collection of eggs
  • Obtaining the sperm
  • Putting the eggs and sperm together in the laboratory, and providing correct conditions for fertilization and early embryo growth
  • Transferring the embryos into the uterus
Treatment of male infertility is difficult and sometimes frustrating. Immediate results are hard to produce and persistence with therapy is required.


The following modalities of treatment are generally employed.
  1. Medical treatment : - This consists of the administration of certain drugs to improve seminal quality. Clomiphene citrate, mesterolone, tamoxifen, gonadotropin injections, antibiotics, steroids and a whole lot of other medicines etc. are commonly used. In India, there are also many alternative medicines. Except is some specific clinical states, most of these are of dubious efficacy, at best.


  2. Surgical treatment :- Our best IVF doctors have been performing microsurgery for male infertility. Obstructions in the sperm conduction pathway, varicoceles, undescended testes etc. can be treated by operation.

    Modern microsurgical techniques are of great help. Even patients who have undergone a vasectomy in the past can have their vasectomy reversed and the tubes recanalised successfully using microsurgery.


  3. Assisted reproduction : - In many cases, neither medicines nor operations are of help. In such cases, an attempt is made in the reproductive laboratory to improve semen quality and facilitate the penetration of the sperm into the ovum. This includes sperm washing/capacitation, intra-uterine insemination (IUI), gamete intra-fallopian transfer (GIFT), in vitro fertilisation (IVF), and micro-manipulation (ICSI).
